Consent Form for “Add On” Mass Removal Procedures
Use this consent form if your pet is having another primary surgery (such as spay, neuter, dental cleaning and/or extractions) and will also, under the same anesthesia, be having a mass or masses surgically removed.

Consent Form for General Anesthesia
Use this consent form if your pet is being placed under general anesthesia for procedures such as anesthetized examinations, x-ray procedures that require general anesthesia, or other (generally non-surgical) procedures.

Declaw Surgery Consent Form
Use this consent form if your cat is to be having surgery to be declawed.

Dental Procedures Consent Form
Use this consent form if your pet is having dental cleaning, tooth extractions or other dental procedures as their primary procedure while under general anesthesia.

Mass Removal Surgery Consent Form
Use this consent form for mass removal surgeries if the ONLY surgery that your pet will be having is mass removal.
