Cat Boarding Form
On rare occasions, animals can become ill while being boarded in our clinic. In an effort to provide your pet with the very best possible care while you are away, we require that you complete the information below. In case of potentially life-threatening situations, or in case of potentially very expensive treatment, we will attempt to notify you through the person and/or phone number listed below.
- We require that any animals boarded be current on routine physical examinations, including a microscopic examination of the feces for parasites. Unless proof of such tests can be obtained from another veterinarian, the tests will be performed prior to boarding.
- Animals must be current on all vaccinations. Unless proof of vaccinations can be obtained from another veterinarian, the pet will be vaccinated prior to boarding.
- Animals with severe flea and/or tick infestations will be treated at the owner's expense ($35-$40 depending on the size of the animal and degree of infestation).
These policies are for the well-being of your pet as well as the well-being of other pets in our facility. Thank you for your cooperation.
